Your appointment request
The form collects your name, email, phone number and treatment choice so Hayden can respond and arrange a visit. Submitting it does not automatically confirm an appointment. It does not collect health information. Please don’t send medical history through this form, email or text.
Hosting and service providers
Framer provides this website and its form. Submitted contact details are processed by Framer and the email provider used to receive them. Hayden and people authorised to help administer his practice may have access. These providers may process information outside Québec or Canada. Clinics use their own booking services and privacy policies.
Browsing and maps
Framer and its providers may process technical information, including IP addresses and usage data, to operate and protect the site. Maps are static images and do not request your location. Opening Google Maps, social profiles or other external links takes you to providers with their own policies.
Health questionnaire
Blank PDFs can be downloaded, completed on your device or on paper, and brought to Hayden. This site has no field to upload a completed questionnaire. Please discuss appropriate ways to share clinical information directly with Hayden.
Retention and your choices
Information is retained as needed to handle requests and meet applicable obligations. Ask Hayden which retention period applies to your information. You may request access, correction or an explanation of its use and, where applicable, withdraw consent or request deletion, subject to retention obligations. If a concern remains unresolved, you can contact Québec’s Commission d’accès à l’information.
