Requests and confirmation
The form is for independent appointments and home visits. It sends a request; Hayden confirms the location, length, price, receipt options and appointment directly with you. The service-area map is indicative and does not guarantee coverage of every address. Book clinic appointments directly with the clinic.
Cancelling or rescheduling
Please give Hayden at least 24 hours’ notice for independent appointments and home visits. If you are ill or something unexpected happens, let him know as soon as possible. This policy imposes no cancellation or no-show fee. Clinic appointments follow the clinic’s policy.
Insurance and reviews
Reimbursement depends on your plan and treatment. Ask your insurer to confirm coverage and recognition of Hayden’s qualifications. No reimbursement is guaranteed.
